Do I need an appointment to see Dr. Appleton?+
Yes. Patients are seen by appointment to ensure you receive the time and attention you deserve. Same-day appointments may be available depending on the schedule. Please call our office during business hours to check availability.
What should I bring to my first visit?+
To help us prepare for your appointment, please bring:

  • Any recent imaging (X-rays, MRIs) or lab results
  • Completed new-patient forms, if available
Arriving 15 minutes early helps ensure a smooth check-in process.
Do you accept my insurance?+

What forms of payment do you accept?+
We accept cash, checks, debit cards, and all major credit cards. Co-pays and any outstanding balances are due at the time of service.
Do you see self-pay patients?+

Can I bring imaging from another facility?+
Absolutely. If you've had X-rays, MRIs, or CT scans done elsewhere, please bring the images (on a disc or digital copy) and any related reports. This helps Dr. Appleton make the most accurate diagnosis.
Do I need a referral to make an appointment?+
Some insurance plans require a referral from your primary care provider. If you're unsure, your insurance company can confirm whether a referral is needed.
Where do I park?+
Our office is located within a medical facility that offers convenient on-site parking. If you need assistance getting from your vehicle to the office, please let us know when scheduling.
How long will my appointment take?+
Appointment times vary depending on your condition and whether imaging or additional testing is needed. Most visits last between 30 and 60 minutes.
